If you are attempting to log on or move your subscription to a new device, but are receiving an error message, use this form to reset your subscription’s security token.

Please enter a valid mobile phone number, which will be used to send you a confirmation text message regarding your request.
This is a unique one-to-five digit number assigned to your subscription by StreetWise when it is issued. If you do not know your Device ID number, contact your department's StreetWise Project Liaison or call StreetWise Support at 800-718-8027.
Your License Key is an alphanumeric passcode and is case sensitive. If you do not know your License Key, contact your department's StreetWise Project Liaison or call StreetWise Support at 800-718-8027.
